The First Choice Health Employee Assistance Program is a division of First Choice Health, a Northwest
based company that was formed in 1984 and is owned by eleven hospitals and approximately 350 medical
clinics and physicians.
Over the years, our EAP has expanded geographically to provide service throughout the United States and
in eleven foreign countries. Our Provider Network is extensive and ensures convenient access to services
regardless of location. You will find our staff is dedicated to excellence in client care and customer

First Choice EAP is a full service program with options for multiple visit models of service delivery
and a full range of Work Life Programs.
