First Choice Health Home : Employee Assistance Program : Provider Information
First Choice Health Employee Assistance Program has received an overwhelming response
to our recent efforts to recruit new providers. As such, many of the areas which we currently
serve are no longer in need of additional EAP network providers. Therefore, as of June 1, 2009,
we have reduced our efforts to bring new providers into the EAP network. While we will still
be accepting new applications, only licensed providers who are in specific geographic areas,
or who offer specific services will be considered.
Qualifiers for joining the EAP Provider Network:
Location by State:
(Not all areas are in need of additional providers)
- Alaska
- Idaho
- Montana
- Oregon
- Washington State
Specialties and Criterion:
- Fluency in languages other than English
- CISM experience
- SAP Providers
- Skilled in EMDR
- Chemical Dependency Professionals
- Children’s issues
- Sexual abuse
- Contracted with Insurance networks (a necessity)
- Available evening hours
If you meet the criteria for any or all of the above, you are encouraged to apply.
If you are interested in learning whether you may be eligible to join the network,
please send an e-mail to feedback@FirstChoiceEAP.com,
using the subject line "Provider Request to Join the EAP Panel." Please include your Name and
Credentials, Specific Location, Contact Number, Email address, and which of the above
criteria you meet. If you are a licensed provider who can help fulfill the remaining
needs of our network, you will be sent an EAP Application and Contract.
First Choice Health Employee Assistance Program regrets that not all licensed
providers who apply will be accepted into our preferred EAP network, but we do
appreciate your continued interest. Should the needs of our network change, you
will be advised of any updates through this website.
