First Choice Health Home : Employee Assistance Program : Provider Information

First Choice Health EAP and its Provider Relations Department is always pleased to admit and credential new providers into our Network. Naturally, we have greater needs in some areas of the country than in others, and from time to time we do close our Network to new applicants.

We are always attempting to identify providers with expertise and skills in treating age-specific groups (adolescent, elder, children, etc.) with fluency in languages other than English, in sign language, and additional cultural expertise. It is always beneficial if you are trained in Critical Incident Stress Management (CISM), are a Substance Abuse Professional (SAP) or Certified Chemical Dependency Counselor, have training in Mediation, or enjoy public speaking and delivering educational trainings.

If you are identified as a candidate for our EAP Network, you will be evaluated by our Provider Relations Department in strict accordance with the standards adhered to by the First Choice Health Network. This certification includes source verification of the material you submit and potential site visits to your place of practice.
