Direct telephone access to our FCH MM team provides you with support for Pre-authorizations and Provider Reconsiderations. Our website allows Providers to access a variety of
forms to assist with pre-authorization and referrals.
First Choice Health Medical Management (FCH MM) provides services to all of First Choice Health Administrator (FCHA) groups and select First Choice Health PPO Network (FCH PPO) Payors. Providers can contact FCH MM by referring to a members' ID card, or by calling (800) 808-0450 to obtain pre-authorization requirements.
