First Choice Health PPO Network (FCH PPO) is provider owned. This means that through our highly qualified leadership team we strive daily to meet the sensitive needs of our providers. Since 1985 it has been our top priority to support the network and ensure provider satisfaction remains central to our success.

Our online tools allow providers to locate addresses for submitting claims and telephone numbers for benefit and eligibility information. Additional online resources are available to assist providers when communicating with patients and FCH PPO.

As a subscriber to One Health Port you have access to the following tools using your OneHealthPort credentials:

List of Payors: Downloadable document containing payors accessing FCH PPO, who to call for claims status, eligibility and benefits
Priced Claims Status: View a priced claim when sent to FCH PPO’s PO Box for pricing
Claims Activity Reports
Employer Group List: Downloadable document (snapshot) of the most current group information for the corresponding month.
