As a First Choice Health PPO Network (FCH PPO) provider you are a preferred provider that serves insurers, large employers, unions and other sponsors of health benefit plans that provide an incentive to their members to utilize FCH PPO. The FCH PPO logo appears on the member ID card along with the name of the sponsoring employer, insurer or union.

Identifying Insured Members

The First Choice Health PPO Network (FCH PPO) logo will appear on the member's identification card along with the name of the sponsoring employer, insurer or union.

How to File a Claim as a Network Provider

Each member's Identification Card indicates the address to which claims should be sent. If you have any questions about submitting claims, please call the Provider Relations Department at (800) 231 6935 Ext. 2103 or ProviderRelations@fchn.com.

First Choice Health PPO Network (FCH PPO) cannot provide specific eligibility, coverage or claims information. If you need a benefit & claims phone number, claims address, or pre-authorization phone number, please see our List of Payors. A Payor is a contract holder, such as an insurer or a third party administrator, with FCH PPO that sponsors employer groups' access to our network of providers.
