Payor - Employer Group Search
Below is a search function for First Choice Health PPO Network (FCH PPO) Payors, Employers and Groups. This section will enable you to locate addresses for submitting claims, and telephone numbers for benefit and eligibility information. You can search by Payor Name, Employer/Group Name or by Employer/Group Number.
If the Payor, Employer or Group accesses FCH PPO, this search function will provide you with a claims mailing address and a benefit phone number for the Payor.

Please Note: this search function does not include the ability to search
by union local trust numbers, unless the local trust number is referenced
on the ID card as the "group number."
As a subscriber to One Health Port you have access to the following tools using your OneHealthPort credentials:
- List of Payors: Downloadable document containing payors accessing FCHN PPO, who to call for claims status, eligibility and benefits
- Employer Group List: Downloadable document (snapshot) of the most current group information for the corresponding month.
