As a First Choice Health Network provider you are a preferred provider that serves insurers,
large employers, unions and other sponsors of health benefit plans that provide an incentive to
their members to utilize the First Choice Health Network. The First Choice Health Network logo
appears on the ID card along with the name of the sponsoring employer, insurer or union.
Identifying Insured Members
The First Choice Health Network logo will appear on the member's identification card along
with the name of the sponsoring employer, insurer or union.
How to File a Claim as a Network Provider
Each member's Identification Card indicates the address to which claims should be sent.

First Choice Health Network cannot provide specific eligibility, coverage or claims information.
If you need a benefit & claims phone number, claims address, or pre-authorization phone number,
please see our List of Payors. A payor is a contract holder, such as an insurer or a third party
administrator, with First Choice Health Network that sponsors employer groups' access to our network
of providers.
