Below is a search function for First Choice Health Network’s payors and
employer groups. This section will enable you to locate addresses for
submitting claims and telephone numbers for benefit and eligibility
information. You can search by Payor Name, Employer/Group Name or
by Employer/Group Number.
If the group or payor accesses the First Choice Health Network,
this search function will provide you with a claims mailing address
and a benefit phone number for the payor.

List of Payors (Yellow Pages)
You can download our most current List of Payors which contains additional
information about claim addresses, EDI submitter numbers and telephone numbers
for payors. If you are inquiring about an address for submitting a Hospital
claim, please note that your claims addresses may differ. The addresses
contained in this portion of the website may not be specific to your contract.
Employer Group List (Blue Pages)
You can download a monthly updated copy of our Employer Group List which lists
Employers by name, group number and their effective date with First Choice
Health Network. This is a snapshot of the most current group information for
the corresponding month. For the most up to date information, please use the
Payor and Employer group search above as it contains "real time" data.
If you would like to receive the Employer Group List in Excel format via email to sort the fields
in this report, please contact Provider Relations at 800.231.6935 extension 2103 or via email at
ProviderRelations@fchn.com.
Important
Please note: this search function does not include the ability to search
by union local trust numbers, unless the local trust number is referenced
on the ID card as the "group number."
