Who is First Choice Health
First Choice Health (FCH) is a trusted physician and hospital-owned company that serves the United States, and has provided network, administrative, and client services since 1985. FCH will handle the processing of your medical claims and inquiries related to your medical plan benefit and provider network.
How do I connect with FCH?

Will I get a new insurance card?
Yes, each enrolled person will receive a combined medical and pharmacy ID card prior to X date. If you don't receive a new ID card by X date, contact FCH Customer Care by calling 888-896-1115. Please update your ID card information with your physician and pharmacy once received.
What if I'm in the middle of care or have an upcoming surgery?
If you have a planned surgery or procedure during this transition, the FCH Case Management team is available to support you navigate your diagnosis and care needs. Connect with them directly at 800-808-0450 or by email at [email protected].
Can I view my benefits and claims information online?
Yes, you have 24/7 online access to your benefits and claims information on the FCH member portal: myFirstChoice. Register for the myFirstChoice member portal.
