Understanding Your Network Tiers
When selecting your medical plan, it is important to understand the plans being offered to you and the network tiers. The following applies to each region.
MyConnected Care and High Deductible Plans
The MyConnected Care plans and High Deductible plans include the provider networks below. The tiers are listed from lowest out-of-pocket costs (Tier 1) to highest out-of-pocket costs (Tier 4). Choosing a provider in Tier 1 will be the least expensive for you and your family. You can access care within other tiers, but it will be at a higher cost.

Standard PPO Plans
The Standard PPO benefit plans include the following provider networks. The network tier structure is listed from least expensive to you (Tier 1) to most expensive (Tier 3). Choosing a provider in Tier 1 will generate the lowest out-of-pocket expenses for you and your family. You can also access care within other tiers, but at a highest cost.
