Claim Reimbursement Forms

Medical Plan Reimbursement

If you need to submit a claim for reimbursement, please remember to keep your original receipt(s) as they will be required with the submission. 

To submit claims for reimbursement online with First Choice Health, follow these steps:

1. Log in to your myFirstChoice portal
2. Select 'Claims' from the left side navigator
3. At the top of the screen, click the link under 'Claims Self-Submission'

Pharmacy Plan Reimbursement

If you need to pay out-of-pocket for a prescription, you will need to complete a reimbursement form and submit to Ventegra along with the original prescription receipt(s). Claim forms are available below.
